Documents for simple ID card application?

Very simple question... nothing to do with eligibility, my parent's nationality, length of stay, number of stars - nothing like that It's a bog-standard non-permanent ID for a foreigner with a work visa.

Just... When I go along to apply for a card, which documents do I need to take with me? My UK passport with activated-on-Macau-trip visa, obviously, but what else? Offer letter from employer?

Can't see the answer anywhere on the Immigration Dept. website - I've emailed them but I've got no idea how long it might take to reply, and I'd guess there are a few people here who've done it...

If you read before posting you would see that the OP has already looked on the ImmD website. The problem is that the form they require (ROP138) is not available online.

And that the OP went to do the deed last Tuesday

In the end, I did call - thanks, db, it was the right answer - and they told me all I needed was my passport with my visa in it, and they were right.

And no, I don't know why you can't download the form, even if it's with a big INVALID watermark over it or something, so you can at least see it before you go. On the other hand, it's a pretty painless process, as far as big faceless autocratic bureaucracies go
