I Know this doesn't exactly answer your questions, but here's some info which may help you compare:

I've bought travel insurance in HK for visiting relatives (not from the US) from AIU: http://www.aiu.com.hk/info.html . Pricing is all on the website which also contains HK sales hotline number. Very straighforward process. Wording is a bit oriented towards travel originating from HK, but sales people confirm it covers incoming travellers too. (Haven't had occasion to test claims process).

Most HK insurance policies limit insurance for HK residents with HK ID card only.
She better gets it in the US before she comes.

Not the case with the AIU one I mentioned. And speaking of hassles, it was the easiest possible thing to do from HK. Just studied the conditions and premia online, asked sales a couple of questions (e.g. re coverage of non HKers visiting HK and other places in Asia - yes counts as single trip till they return to home country), faxed back completed downloadable appl and credit card debit authorization, and cert was issued and recd as e-mail attachment same day. Coverage was in place before departure from home country and covered began with that leg of journey. No medical questionnaire needed. Coverage above a certain age (70?) was automatically 50% of all the individual item limits.

AIU/AIG/AIU: all part of American International of the U.S. - HK office is in AIA Bldg in Causeway Bay. Worth giving them a call, at least to be able to compare.
