Legally it is you who applies for the Employment Visa, with supporting documentation from your sponsor (i.e. your potential employer). In practice companies which employ a number of foreigners tend to have HR people who will help you through the process to a greater or lesser extent. Big multinationals tend to say "give me your passport, educational certificates, and sign there" and then sort it all out. But in a small local company where you are the first foreigner they have ever employed then you'll probably be largely on your own.

I believe the fee (HK$135 I think) is paid when you collect the visa.

I'm not sure you understand that to apply for a work visa you MUST have a job offer from a company in Hong Kong, and that company must provide quite a bit of supporting documentation to justify why they want to employ a foreigner... You cannot simply send a form in saying "Please give me a work visa".

Those questions must come from companies that have never sponsored an Employment Visa before. The actual taking of the form to Immigration and going through the logistics of it is the easy bit. Writing the supporting letters and providing the supporting documentation is far more time consuming. There is no way the company can avoid being involved in that.

What they mean may actually be: are you willing to pay the costs involved in processing a visa by an agent?
