US Passport/HK Re-entry

I have a US passport so I won't need a visa to HK.
I will be going to China so I know I will need a re-entry visa.
What is the name of this form?
Is this it: www.immd.gov.hk/ehtml/hktraveldoc_2_rp.htm

What else will I need? I have HK permanent residence if that makes a difference.

HK permanent residence is a REQUIREMENT for that purpose..

If you have permanent residence, doesn't that mean you also have a permanent ID card in which case you won't even need a passport to re-enter HK, let alone a visa.

Point of note. If you are going to visit Mainland China a few times over the next few years, you are better off getting a 3-year multiple entry visa for China, though I'm afraid US and UK passport holders pay 3 times more than anyone else (about HK$1600)

3 year multi-entry? I have seen ways to get a 2 year, but never a 3 year. Do you have a website in mind for that?
I was going to shoot for the 2 year this next trip, but waited too long to get all the documents/letters together. Maybe next time. Plus the site I was using said I had to have at least 2 entries on an F visa in the past 6 or 12 months before I could even think of applying for a multi-entry.

I got myself a 3 year multiple visa with my British passport in 2003, and a mate of mine (also British) also got one this year. He regularly crosses into Shenzhen on day trips with it.

I was told at the time that you need a HK permanent ID card to qualify for this, and each visit is limited to 30 days (I'm not sure whether or not it's as easy to extend as a normal tourist visa). you can get this at the Chinese foreign ministry in Wanchai or through a CTS branch here in HK.

And you think Americans can get the same thing? The number of visa rumors is astounding. Not that what you are saying is a rumor, but if you look at all the visa threads it makes you think the Chinese make this up as they go along.

As I understand it, anyone with a HK permanent ID card can get this visa regardless of Nationality. British, and I heard also Americans, are now discriminated against in that we pay 3-4 times more than anyone else. I was in CTS only yesterday and was told that renewing this visa would cost me HK$2040 (I paid around $600 in 2003).

I'm surprised if this is not already well known by users of the Geoexpat travel forum.

I imagine that people know that, but when you read through all the visa info you get the impression that the answer/cost depends on where you go, when you go and the mood of the person you are seeing.
